You must have a Medicare card to receive this vaccine at Southgate Medical.

There must be 7 days minimum between your COVID19 vaccine and any other vaccines.

It is encouraged that you receive your 2nd COVID vaccine dose at the same clinic you received your 1st dose.

You MUST be in this age bracket to book this appointment. If you believe you are eligible, please bring any eligibility documentation or be prepared to sign a self-declaration eligibility form on the day.

Please remember to wear a loose-fitting shirt and be prepared to wait 15mins post-vaccination for observation.

If you no longer require your appointment, please phone the clinic to cancel so that we can offer the vaccine to someone else.
