This is a private billing service. Payment is required at the time of request.

✅ Only for existing patients who have attended Kogarah Family Practice within the last 12 months.

❌ Not for WorkCover or fitness for work certificates.

❌ Not for certificates for children under 16 years.

🗓️ Medical certificates cannot be backdated and are issued for one day only on the date of the request.

📅 If you require a certificate for multiple days, or for a child under 16 years, please book a GP consultation instead.

🩺 Each request is reviewed by a doctor and approved only if clinically appropriate.

No refunds will be issued for change of mind.
