Herald Avenue Family Practice is a fully accredited, privately owned medical centre that aims to provide professional and personal care to patients. Having opened in November 2000, we are a well-established practice and operate from purpose-built premises, equipped with the latest medical equipment and technology.

Our friendly, welcoming staff work together to provide an environment that ensures both patients and visitors are given the highest level of service at all times, whilst incorporating a holistic approach toward the diagnosis and management of illness. We offer a wide range of general practice services, including:

• General health assistance
• Nursing services
• Skin cancer checks
• Minor skin surgery
• Health assessments and care plans
• Immunisations.

We also offer onsite pathology services through Pathology who are co-located with our practice. A dental centre is located adjacent to our practice, should these services be required. A variety of additional health services are conveniently located nearby, including pharmacy, radiology, physiotherapy, chiropractic, podiatry, psychology, and dental services.

PLEASE BE AWARE :

As a mixed billing practice, we offer bulk billing at the discretion of the treating doctor. Bulk billing may also be limited to certain hours. In addition there is no bulk billing available on Saturday.

For further information, please enquire with our friendly reception team or your treating doctor. In general, Dr Ji Li Tan and Dr Daniel Lim will BULK BILL children under 16 and patients with a valid concession card.

Conveniently located adjacent to Riverton Shopping Centre, we are ideal for residents of Willetton, Bull Creek, Rossmoyne, Shelley, Riverton, Parkwood, Canning Vale, and Leeming. We are close to public transport services and there is ample parking available onsite for those who prefer to drive.

Appointments are available 6 days a week, from 8am to 6pm Monday to Friday and from 8am to midday on Saturdays. Please note that we are closed on public holidays.
