Clovercrest Family Practice was established in April 2006 and is located within the Clovercrest Village Shopping Centre in Modbury.

Our experienced health professionals are committed to providing high quality, personalised care. Our skilled nursing team works closely with GPs to support treatment and promote overall wellbeing. We take the time to understand each patient’s needs, ensuring tailored care at every stage of life.

ADHD Assessments Now Available. Dr Eman now provides comprehensive ADHD assessments for adults and children aged 8 years and over.

Our team offers a comprehensive range of services, including: 

·         General Health; routine consultations & medical care for all ages.

·         Women’s Health; cervical screening (pap smears), Implanon insertion & removal, Mirena removal, menopause management, & contraceptive advice.

·         Men’s Health; preventative care & management of health issues specific to men.

·         Newborn and Paediatric Health; health assessments & ongoing care for infants & children.

·         Preventative Healthcare; health assessments, risk factor identification, & early intervention.

·         Chronic Disease Management; ongoing care & support for patients with chronic conditions.

·         Family Planning and Reproductive Health; antenatal shared care, post-natal care, contraception, & sexual health services.

·         Childhood Immunisations; routine immunisations in line with the national schedule. 

·         Travel Medicine and Vaccinations; travel health advice & Yellow Fever Vaccinations. 

·         Mental Health Support; mental health care plans, counselling & ongoing support.

·         Iron Infusions; intravenous iron therapy for the treatment of iron deficiency.

·         Skin Checks and Cryotherapy; comprehensive full body skin checks & treatment of lesions using cryotherapy.

·         Minor Surgical Procedures; wound suturing, skin lesion removal, & other in-clinic procedures.

·         Ingrown Toenail Management; wedge resection.

·         Wound Assessment and Care; management & dressing of acute and chronic wounds.

·         Occupational Health; Return To Work, pre-employment medicals & driving medical examinations.

·         Diagnostic Testing; ECG, spirometry, & ABI assessments.

·         Ear Syringing; safe & effective ear wax removal.

·         Nursing Support Services; our skilled nursing team provides essential support & care across a range of clinical services.

·         Allied Health Services; Podiatry, Physiotherapy & Dietitian.

·         Pathology Services; ACL are available on site.

ALL PATIENTS MUST PRESENT A VALID CONCESSION CARD TO EACH APPOINTMENT FOR BULKBILLING (Monday-Friday)

• Gold DVA Cardholders

Some consults may incur a gap fee and will be at the general practitioner's discretion.

ALL SATURDAY CONSULTATIONS WILL INCURE A GAP FEE

PATIENTS WITH NO CONCESSION will incur a gap fee for consultations.

Various Procedures, dressings, iron infusions, yellow fever vaccinations will incur a higher out of pocket expense.

Payment to be made on the day of your appointment. NO ACCOUNTS WILL BE ISSUED.

Medicare Rebatable telehealth appointments are available with your regular GP if you have had a face-to-face consultation in our practice in the last 12 months. These appointments do incur a gap fee.

Patients without a registered Medicare card will be charged from $95.05for face-to-face & telehealth consultations.

Dr Chris Teh privately charges all patients, contact the practice for Dr Teh’s consulting fees & booking appointments.

We are located nearby to public transport, have ample parking available and full wheelchair accessibility to the practice.

Appointments are available Monday - Friday 8.00am – 5.30pm. Saturdays 8.30am – 1.00pm.

We are closed on Sundays and Public Holidays.
